Durable Power of Attorney for Health Care
Better than a living will, this is an attorney drafted style power of attorney for health care. It gives an agent the power to make health care decisions upon the determination of the agent and the treating physician that the signer has become incompetent as defined by the California Probate Code. It provides for termination of medical treatment in case of coma or an incurable condition. This version limits the agent from authorizing an autopsy, gifting body parts, etc. but all clauses can be modified to suit individual preference. Available in WordPerfect by email request.$20.00
